Describe the Problem: Provide a detailed description of the problem, stating it in observable and measurable terms. Include information regarding when and where the problem occurs, how often and for how long
Interventions: Describe the instructional interventions and strategies that have been provided to address the problem. (e.g., Elkonin boxes, repeated readings, folding in, math fluency drills, social skills group 2 x’s wk, video modeling, etc.)
Accommodations: List the accommodations that have been made available to the student and when the accommodations are provided. (e.g., preferential seating, prompting, redirecting, visual schedule, etc.
